As a POC Nurse Coordinator, your primary responsibility will be to ensure adherence to quality standards and regulatory requirements. Your duties will include, but are not limited to, the following:

1. Quality Assurance Review:

· Review and approve the daily Quality Assurance (QA) reports to ensure compliance with established standards and protocols.

· Identify any discrepancies or issues and collaborate with relevant stakeholders for resolution.

2. Plan of Care (POC):

· Ensure that PDN patients are recertified every 60 days in accordance with company policies and regulatory guidelines.

· Conduct recertification and update Plan of Care (POC) for PCA patients every 60 days, adhering strictly to company protocols.

· Ensure that PPEC patients are recertified every 180 days in line with company and regulatory requirements.

· Coordinates the completion of Plan of Care in an accurate and timely manner, in accordance with policies and procedures, State and Federal requirements.

3. Start of Care (SOC):

· Ensure that Start of Care (SOC) assessments are performed when the patient has obtained insurance authorization, in line with regulatory requirements.

4. Document Administration:

· Complete the Plan of Care (POC) documentation accurately and promptly within the Axxess system or Acentra/EQ for authorization.

· Obtain signatures from family members on the company’s contract document during patient visits.

· Effectively communicates with Director of Nursing Clinical Managers, Doctors etc., involved in the care planning process.

· Conducts regular reviews of patient records.

· Responsible for managing the administrative components of patient care plans.

· Perform administrative duties such as completing medical forms, reports, etc., as necessary.

· Any other duties that may be assigned.

Qualifications:

· Must possess a current, unencumbered, active license to practice in the state of Florida.

· Must have pediatric experience.

· Strong understanding of healthcare regulations and compliance standards.

· Proficiency in using healthcare software systems, particularly Axxess.

· Proficiency in Microsoft suite

· Excellent communication and interpersonal skills.

· Detail-oriented with a commitment to accuracy and thoroughness.

· Ability to work effectively both independently and collaboratively within a team environment.
